ADMISSION OF PATIENT- A PROCESS TO INITIATE QUALITY CARE IN HOSPITAL

What is admission of patient in hospital ?
admission of patient in the hospital is the entry of patient either for preventive, pro motive, curative or rehabilitative utilization of health care facilities.

Types of Admission :

There are two major types of hospital admissions, emergent and elective.

Emergent admission usually happens when a patient seen in the emergency department is subsequently admitted to the hospital. Eg : Heart attack, Stroke, Burns

Elective hospital admissions occur when a doctor requests a bed be reserved for a patient on a specific day. The patient then checks in at the admissions office and does not go to the emergency department. Elective admissions make up the majority of hospital admissions, but this percentage varies markedly with some hospitals.  Eg : Planned surgery for LSCS, TKR etc

Other types are :

1. Observation admission

2. Direct admission or telephonic admission , discuss with the consultant & get admitted.


Parameters that greatly influence Admission Process:

A. Time Taken at Admission Counter

B. Time Taken for Insurance

C. Time Taken at Cash Counter

D. Time Taken for Bed Allotment

E. Waiting Time for Miscellaneous Reason


Delays during Admission :

  • Mostly the delay occur because of the approval status of insurance as in case of planned admission approval of " request for authorization approval " from the company is pending as it is to be send 7 days prior to admission by the patient by providing necessary details of treating doctor & treatment procedure
  • Sometime the delay occur because of the unavalibility of the bed in the hospital ,at that time ,the patient has to wait in the emergency dept. 


Admission Procedure involves :-

  •        Authorization from a physician
  •       Collecting of billing info from the admitting dept.
  •       Completion of the admission process by nursing.
  •       Documentation of patient medical history,physical exam
  •       Initial medical orders for treatment.

Role of Admission department of Hospital during Admission process:

  •       Gather information from billing
  •       Intiate medical record
  •       Prepare id bracelets. ( this is the single most effective way to identify the patient. )
  •       May bypass the emergency situation.
  •       An addressogram id card is made.


Roles of Health care professionals :

  • Identify & greet the patient.
  • Explain Patient about norms of hospital
  • Carry out baseline Assessment of patient
  • Get the consent forms signed
  • Administer medications if order is stat
  • Explain about the procedural steps in case of any diagnostic or surgical procedure
  • Carry out necessary pre-procedural steps before any procedure
  • Ensure privacy of patient is maintained
  • In case of emergency carry out all procedures & medications without delay
  • Opt therapeutic communication & relation with the patient
